Screen4Success is a user-friendly screening tool for you and those you care about. A Personal Account is specifically for individuals (i.e., yourself, parents, caregivers, legal guardians, etc.). It will allow you to self-screen, take a screener with someone you care about, or send them a screener to fill out on their own. To sign up for a Personal Account for Screen4Success, please enter your email address, select “I understand,” check the validation box, and press send.
