Screen4Success is a quick and easy way to identify areas where someone may benefit from more support. It also provides local and national resources to help address those concerns. You can use the tool for self-screening, or you can send it to someone you are concerned about. You can also help that person fill out the screener—this provides opportunities for discussion in the moment—or they can complete it on their own if that’s not possible.

Early identification and screening are among the best ways to protect your health, wellness, and well-being. Screening serves as a fast and easy way to identify areas of concern or areas where someone may benefit from more support. While the screener does not provide a clinical diagnosis, it will identify signs of elevated risk and provide resources to help you address your concerns.

How can you help someone else take a screener?

You can help someone you are concerned about take a screener by filling it out together. This provides opportunities for discussion in the moment. If that’s not possible, you can also send a screener to that person to complete on their own and ask them to share the results with you. The screener results will provide information and resources to help take the next steps in addressing your concerns.

A screener can be taken any time you are concerned about your own or someone else’s health, wellness, or well-being.

Who can take a screener?
Who can take a screener?

Any person older than 12 years of age can take a screener. You can take it yourself or send it to someone you are concerned about, such as a friend, a family member, a student, or your child.

Your privacy matters.

Confidential

Your username and contact information are only for you to track changes over time. They will not be exported or shared. Screen4Success will store your data securely for your use.

Permission

You decide who sees your screener information. Screen4Success will not share any information without consent or permission. You can change your mind at any time.
