This version of Screen4Success allows you and your organization to determine if the youth you serve are at increased risk for mental health and substance use issues and need additional support. Please review the following information prior to completing the request Form.
What is a Professional Account and who is it intended for?
A Professional Account is for community-based health organizations, local or state education agencies, and student assistance programs that work with groups of youth and/or adults. It is not intended for individual parents, caregivers, or families who wish to use the screener with their children. Screen4Success should only be used with the population you serve if an individual shows signs of elevated risk regarding their health, wellness, or well-being.
All Professional Account requests should be made by the leader of the interested organizations.
How is a Professional Account different from a Personal Account?
A Personal Account is primarily for individuals (e.g., parents and other caregivers) who want to complete a screener with someone, send someone a screener to complete on their own, or take the screener themselves. This type of account should be used when you have five or fewer Participants completing a screener.
A Professional Account is for those who work for an organization that serves large groups of youth and/or adults. Using a Screen4Success Professional Account allows you to create Subgroups within your organization’s main Group, assign responsibilities to staff members who can oversee screener dissemination and completion, send a screener to multiple people at the same time, and access a dashboard that features your Group’s permitted screener data.

Is Screen4Success a universal screener?
Screen4Success is NOT a universal screener meant to be administered to an entire school or the general population. It is a brief screening tool that should only be used by a concerned adult when a youth or adult is acting in ways that suggest they may be having problems related to their health, wellness, or well-being. Specifically, Screen4Success is meant to be used with individual youth and adults who are showing signs of elevated risk or dealing with mental health or substance use issues.
Screen4Success helps identify individuals at elevated risk for potential problems.
What does Screen4Success screen for?
Screen4Success asks questions about substance use, mental and physical health, general well-being, and family life.
Why is screening important?
Early identification and screening are among the best ways to protect your health, wellness, and well-being. Screening serves as a fast and easy way to identify areas of concern or areas where someone may benefit from more support. While the screener does not provide a clinical diagnosis, it will identify signs of elevated risk and provide resources to help you address your concerns.
How can you help someone else take a screener?
You can help someone you are concerned about take a screener by filling it out together. This provides opportunities for discussion in the moment. If that’s not possible, you can also send a screener to that person to complete on their own and ask them to share the results with you. The screener results will provide information and resources to help take the next steps in addressing your concerns.

A screener can be taken any time you are concerned about your own or someone else’s health, wellness, or well-being.
Who can take a screener?
Any person older than 12 years of age can take a screener. You can take it yourself or send it to someone you are concerned about, such as a friend, a family member, a student, or your child.
Your privacy matters.
Confidential
Your username and contact information are only for you to track changes over time. They will not be exported or shared. Screen4Success will store your data securely for your use.
Permission
You decide who sees your screener information. Screen4Success will not share any information without consent or permission. You can change your mind at any time.
